CONDUCT – All participants in our programs are expected to behave in a manner consistent with our values of respect for others. Unsatisfactory conduct toward others can result in a participant’s removal from our summer and school year programs.

REFUND – It is understood that there will be no refunds of tuition or fees in the event of absence or withdrawal of any participant for any reason whatsoever, regardless of illness, voluntary withdrawal, or conduct deemed unsatisfactory by the school.

REGISTRATION FEE – There is a one-time, $25 registration fee per camper for Big Blue Summer Signature programs.

CHANGE FEE – For any changes or transfers there will be a $25 administration fee per transaction.

NON-DISCRIMINATION – The Pingry School welcomes students of all backgrounds. All students are entitled to the rights, privileges, programs, and activities generally accorded or made available to students at this school. The Pingry School does not discriminate on the basis of race, color, gender identity, and national or ethnic origin in the administration of its educational policies, admissions policies, scholarship and loan programs, and athletic and other school-administered programs.

PARTICIPATION – I certify that my child is emotionally ready and in good health with my permission to participate in this program. I certify that my child has no ailments or disabilities that would prevent my child from participating in this program.

LIABILITY – I understand there is some risk in this program, and I am willing to assume the risks. I hereby agree to hold The Pingry School harmless from any and all injury or illness incurred by my child during participation in this program.

PHOTO/VIDEO RELEASE – Photos and videos will be taken of my child to be used in the Big Blue Summer Literature, including our Facebook and website (will not include any personally identifiable information such as: name, date of birth or address).

MEDICAL – I understand that my child is not permitted to bring to camp any medications with the exceptions of sunscreen, bug repellant, and prescription emergency medications such as those used for the treatment of anaphylaxis or asthma. If I choose to supply my child with sunscreen and/or bug repellant, I understand that camp staff has my implied consent to permit my child to self-administer the provided sunscreen and/or bug repellant, and to aide my child in applying these medications should my child require assistance. Prescription emergency medications are permitted only when noted on an Emergency Medications Form and must be properly labeled with a prescription label and/or doctor’s note. No other medications will be provided or administered by the camp.

PERMISSION TO TREAT – This health history is correct and accurately reflects the health status of the camper to whom it pertains. The person described has permission to participate in all camp activities except as noted by me and/or an examining physician. I give permission to appropriate medical staff to order x-rays, routine tests, and treatment related to the health of my child for both routine health care and in emergency situations. If I cannot be reached in an emergency, I give my permission to the proper medical staff to hospitalize, secure proper treatment for, and order injection, anesthesia, or surgery for this child. I understand the information on this form will be shared on a “need to know” basis with camp staff. I give permission to photocopy this form. In addition, the camp has permission to obtain a copy of my child’s health record from providers who treat my child and these providers may talk with the program’s staff about my child’s health status.
